WATERFALL WELLNESS MOBILE CLINIC

At City Hall the Fourth Wednesday every month from 9AM – 3PM

Services include free flu shots, full primary care visits and minor urgent care evaluations. Other services include Oregon Health Planning assistance or renewal, behavioral health, and social health such as transportation, housing and food issues.

All major insurance companies are accepted as well as self-pay on a sliding income scale and a prompt pay discount.  Appointments and walk ins are both accepted.
